Charles Tan

Charles Tan, PT, DPT

Clinic Director

Charles Tan is a licensed Physical Therapist with over 14 years of experience in patient care and healthcare operations. He is passionate about building strong relationships, leading teams, and driving growth in innovative care models. Currently spearheading the expansion of senior living outpatient programs in Arizona with Ensign Services, Charles combines clinical expertise with strategic leadership to deliver impactful results. Outside of work, he enjoys woodworking, gardening, and traveling, finding inspiration in creating, cultivating, and exploring new experiences.

Emily McEvers

Emily McEvers-Kunce, OT

Doctor of Occupational Therapy

Emily McEvers-Kunce is a licensed occupational therapist with 3 years of experience within various settings and populations including outpatient neuro rehab, memory care and geriatrics. Emily graduated from Northern Arizona University with her Doctor of Occupational Therapy degree. Through her education and experience, Emily has developed clinical skills to assess physical and functional needs in order to create and implement personalized intervention plans and facilitate collaboration with interdisciplinary team members, clients, families and community staff. Emily is passionate about encouraging participation and independence in meaningful daily occupations by addressing a client's needs across the care continuum. Emily utilizes a client-centered approach to maximize a client's engagement in meaningful activities by increasing their support and accessibility within their home and community environments and promoting their level of safety and independence. Outside of work, Emily enjoys reading, hiking, traveling and spending time with her family, friends and dogs.

Jordan Jones

Jordan Jones, OTA/L

Occupational Therapy Assistant

Jordan Jones is a licensed Occupational Therapy Assistant with experience in both post-acute and outpatient rehabilitation. He has a special interest in neuro rehabilitation, fall prevention, and helping adults age safely in place. Jordan values collaboration and enjoys working with his team to create effective, patient-focused care plans. Outside of work, he enjoys playing basketball, reading, traveling, and spending time with family.

Shelly Rapier

Shelly Rapier, PTA

Physical Therapy Assistant

Shelly Rapier is a licensed physical therapy assistant with over 30 years of experience, driven by passion for helping others to achieve their therapy goals.  She's patient-centered and strives to think outside the box and be creative with her treatment sessions.  Outside of work, she enjoys travel, especially to tropical locations and beaches and learning new DIY projects.  She loves trying new food places and visiting little Jem-like places around the Phoenix area.

Erin Boggs

Erin Boggs, SLP

Speech Language Pathologist

Erin is a certified speech language pathologist. Her six years of experience include positions in skilled rehabilitation, home health, assisted living, and memory care. She especially enjoys supporting people with aphasia and voice challenges. When she’s not working with patients on memory or word finding strategies, you’ll usually find her in yoga class or biking outdoors.

Noah Flores

Noah Flores, DPT

Physical Therapist

Noah Flores is a licensed physical therapist with a Doctor of Physical Therapy (DPT) degree from Midwestern University and a bachelor's degree in Kinesiology from Arizona State University.  With over three years of experience in the field, Noah specializes in outpatient care with a focus on geriatric patients, helping older adults regain strength, mobility, and confidence in their daily lives.

Born and raised in Phoenix, Arizona, Noah is passionate about serving his local community and takes pride in delivering personalized, compassionate care.
